• All candidates for membership are required to successfully complete a background check prior to beginning the membership process. The cost of the background check is $65 and is not refundable.
• You will be asked to read and electronically sign a consent & disclosure form.
• The questionnaire must be completed in one session taking approximately 20-30 minutes. Do not click the link unless you have 30 minutes available.
• When directed to the background check portal, you will be asked for your invitation code. The invitation code is a pre-populated field – do not erase it. Just click “Begin” to start your background check.
• Please be prepared to provide the Chapter Number that you received from the chapter of interest. You will enter this number with your personal information.
• Before final submission, please proofread and verify the information you provided is complete and accurate. NO CHANGES CAN BE MADE AFTER SUBMISSION. Background checks containing errors will require starting the process over beginning with payment.
• Please use Google Chrome or Firefox and NOT Internet Explorer, as the intake form will not work in older browsers.
